This position is primarily based at St. Albans Hospital, focusing on outpatient care, with expectations to deliver clinics cross-site. The core objective is to streamline the diagnostic pathway and provide holistic care for patients at risk of fatty liver disease referred by GPs.
Key responsibilities include:
1. Conduct outpatient clinics for patients with abnormal liver function and other liver conditions, including cirrhosis.
2. Manage MASLD through telephone and virtual clinics.
3. Perform Fibroscans and support the Fibroscan Technician (training provided).
4. Participate in multidisciplinary team (MDT) meetings.
5. Ensure effective liaison with primary care and direct communication with patients.
6. Maintain excellent communication, organizational, and IT skills.
Additional duties:
1. Assess individuals using various methods, including physical exams and diagnostic tests.
2. Work with the Lead Clinician and Lead Nurse to develop new clinical services.
3. Ensure timely action on abnormal imaging.
4. Manage independent clinics for MASLD, ALD, and cirrhosis patients, cross-site.
5. Provide nurse-led telephone clinics for new and follow-up patients.
6. Perform Fibroscan examinations to monitor liver disease severity.
7. Design and implement evidence-based policies and care pathways.
8. Undertake face-to-face and virtual clinics after training.
9. Attend weekly hepatology MDT meetings and present cases.
10. Liaise with hepatology, oncology, gastroenterology, and primary care teams.
11. Maintain accurate patient records and report data.
12. Provide expert clinical knowledge and involve patients and carers in care planning.
13. Establish effective communication with all stakeholders.
14. Promote collaborative relationships and act as a role model.
15. Participate in peer review and acknowledge personal limitations.
16. Support staff development and complete mandatory training.
17. Contribute to surveillance databases and training initiatives.
18. Lead in developing patient information and local monitoring tools.
19. Participate in research and clinical trials as required.
20. Coordinate with CCG groups and escalate poor practice.
21. Manage workload, organize clinics, and attend meetings.
22. Implement service and practice changes in collaboration with management.
23. Contribute to service development and budget planning.
