Job Overview
We are seeking to recruit a highly motivated and passionate individual as a Young Carers Schools Outreach Support Worker in Barking & Dagenham providing support to young carers and school staff with a variety of information, support, informal advocacy, signposting as well as young carers assessments including raising awareness and identifying hidden young carers. Also setting up and facilitating young carers information stands and groups. You will be based at our Heathway office and be part of a busy young carers and adult carer support team including working closely with both statutory and voluntary organisations. Young carers look after individuals with the following illnesses or disabilities: Frail and elderly, mental illness, physical and sensory impairment, a dementia or memory loss, autism or learning disabled. The cared for can be their parent, other adult or sibling.
Responsibilities
* Identify and work with young carers attending secondary schools in Barking & Dagenham.
* Build positive/open working relationships with all secondary schools in Barking & Dagenham
* Assess the needs of young carers referred to the support service
* Provide support, information and advocacy to young carers and their families through Zoom/Teams meetings, telephone support, texts and WhatsApp, support groups, in-house appointments and drop-in sessions
* Case management responsibility for young carers and their family including addressing health, education, emotional support and information needs including counselling referrals and transition to adult services
* One to one work with young carers and their families as required at times that meet their needs
* Refer/signpost to other agencies as appropriate
* Organise and facilitate young carers school drop ins, parents evenings, school assemblies and other young carer focused activities as required
* Assist to prevent NEET for all ages of young carers offering targeted support at all school transitions including to college/university and employment
Requirements
* Strong communication skills and the ability to work effectively within a team environment.
* A valid driving licence will be required
