About the role

The High Support Needs Team is primarily focused on specific student groups, including those with EHC plans who require EOTIS packages or face the risk of placement breakdown in mainstream or special schools, as well as students without EHC plans but with SEND support plans needing longer-term EOTIS provisions while remaining enrolled in a school. Additionally, the team addresses students with complex medical requiring section 19, EBSA, or SEND needs, even if they lack an EHC plan or a SEN support plan, including those requiring hospitalization.

The team acts as the case coordinator, overseeing EHC plan reviews, and provides a range of services such as mentoring, core subjects like English and Maths, and coordinates with other teams or external providers. As an intervention lead you would work directly with the child or young person and their family, this will include visit to their home, school and other venues.

Some of your day-to-day tasks will include:

* Offer direct support to children and young people, delivering provision and supporting engagment
* Make day to day decisions in response to operational issues, accommodating changes in priorities.
* be part of the SEND team to support schools/settings across Buckinghamshire to ensure they are fulfilling their educational responsibilities for children and young people with SEND in line with the SEND Code of Practice, current DfE legislation and initiatives
* Work closely with other agencies and services in a cooperative and holistic way
* Assist staff with monitoring, recording, and evaluating pupils' needs.


About you

We are looking for a dedicated and passionate individual with experience working with children and young people with a range of SEND. You should have a sound knowledge of evidence-based approaches to support school-age children with SEND.

To be successful in this role, you will need:

* A valid driving licence and own transport.
* Excellent interpersonal skills.
* ICT skills, including the use of Microsoft applications and specialist systems.
* The ability to build and maintain effective working relationships at all levels

On-the-go:

Our ‘on-the-go’ roles mean you’ll usually start and finish your day from a base, moving around between different locations throughout the day. You won’t need a fixed desk to work from, but you’ll have access to our network where you can keep up to date with admin and the latest staff updates.
