Overview

Supported Assistant Practitioner/Assistant Practitioner 8727 | Permanent contract | £28,598 to £33,699 per annum (Scale F/G) | 37 hours per week | Norfolk & Norwich University

Could you support the communities of Norfolk using your previous Health or Social Care experience? If so, we’d love to hear from you.

This is a full-time opportunity in our Home First Hub, an integrated team working with Norfolk Community Care and NHS colleagues to ensure safe and timely hospital discharge for individuals in community and acute hospitals across the Central Area of Norfolk. The team supports people to return home, access reablement opportunities and plan ongoing care after discharge.

You will join a 9-month bespoke support programme as a Supported Assistant Practitioner (Scale F) with progression to an Assistant Practitioner role (Scale G) upon satisfactory completion.

You will work in a fast-paced environment with good IT and organisational skills, in roles that may involve visiting hospitals, conducting face-to-face assessments, and office-based work. Some weekend work is required. This is a hybrid role requiring good broadband connectivity.

Responsibilities

* Support safe and timely hospital discharge and coordinate with community and NHS partners.
* Assist in planning and delivering reablement opportunities and ongoing care post-discharge.
* Carry out face-to-face assessments, with the potential to work remotely when appropriate.
* Adapt to changing ways of working and provide patient-centred care in a fast-paced environment.
* Attend the Home First Hub as part of a large-scale transformation to optimise patient experience.
* Maintain clear and accurate information exchange and use community resources effectively.


Qualifications and Skills

* Experience in Health or Social Care sector.
* Good standard of education; excellent IT and organisational skills.
* Ability to work independently or as part of a team; strong listening and analytical skills.
* Flexible and adaptable to new ways of working; commitment to safe hospital discharge.
