Our Acute Frailty team are seeking a highly motivated practitioner with a passion to improve the experience of our ageing and frail population. The post holder must have knowledge and prior experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations. Excellent communication skills are essential to communicate with patients and carers regarding clinical conditions and experience in sensitive discussions around anticipatory care planning would be advantageous. The role may involve working across organisational boundaries and in partnership with other health and social care teams. The successful candidate will join a team who utilise Comprehensive Geriatric assessment as a method of screening frailty and planning multi-disciplinary care. As well as initial screening, the successful candidate will prioritise care and provide daily ongoing medical review of our frail patients alongside the wider multi-disciplinary team. You must be able to demonstrate good organisational skills, excellent communication skills and a commitment to continuing professional development. Shift pattern is variable depending on location. We may also consider applications for trainee posts and will support the development of individuals to meet advanced practice competencies using a triangulation approach of academic courses, professional supervision and clinical practice supervision.
