Our team is passionate about making a positive difference in the lives of our clients and we are looking for a skilled and experienced Senior Care Coordinator to join us in this mission. Responsibilities: Coordinate and manage care services for clients, ensuring their individual needs and preferences are met Develop and maintain care plans for clients, reviewing and updating as necessary Supervise and support a team of care workers, providing guidance and training as needed Conduct assessments and risk assessments for new clients and review existing clients' care needs Liaise with healthcare professionals, families, and other stakeholders to ensure effective communication and coordination of care Monitor and maintain accurate records and documentation in accordance with company policies and regulatory requirements Participate in on-call duties on a rotational basis to ensure 24/7 support for clients Identify and address any issues or concerns raised by clients or their families in a timely and professional manner Contribute to the development and implementation of policies and procedures to improve the quality of care services provided Support the recruitment and onboarding of new care workers, conducting interviews and completing necessary checks and documentationRequirements: Minimum of 2 years' experience in a care coordination or similar role in a domiciliary care setting NVQ Level 3 in Health and Social Care or equivalent qualification Knowledge of complex care needs and experience in managing care for clients with complex conditions Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and healthcare professionals Strong leadership and supervisory skills, with the ability to motivate and support a team Sound understanding of relevant legislation and regulatory requirements in the care sector Proficient in Microsoft Office and electronic care management systems Ability to work flexible hours and participate in on-call duties as requiredIf you are a dedicated and experienced Care Coordinator looking for a new challenge and the opportunity to make a real difference in people's lives, we would love to hear from you.
