Job Role: Targeted Support Worker
Salary: £25,235 per year
Hours: 37.5 hours per week
Contract type: Fixed term contract, until 31st March 2026
Location: Somerset East & Somerset, mainly Frome & Shepton Mallet
Responsibilities
* Conduct needs and risk assessments
* Implement support plans and provide effective support and advice
* Keep client records accurate, clear, and up to date
* Advise and assist clients with budgeting
* Create a safe, supportive and empowering environment that promotes personal growth and sustainable move-on
Role details
We’re looking for a compassionate and proactive Targeted Support Worker to support individuals who have experienced rough sleeping. In this role you’ll work with a small caseload of up to 10 clients, helping them overcome barriers and maintain stable tenancies across local projects. You’ll build strong relationships, offer tailored advice, and participate in evening and weekend work and on-call service.
What We’re Looking For
* Experience working with people who may have complex needs and/or challenging behaviours
* Valid driver’s license and access to own vehicle – business insurance required for roles involving travel
