Job Summary
Are you passionate about helping others? We are seeking a dedicated Community Care Officer to join our Broxtowe Community Social Work Team. In this exciting role, you will help individuals live their best lives in the comfort of their own homes. You will work closely with people and their families to understand their unique goals and aspirations, ensuring they have the support they need to thrive.
Key Responsibilities
* Identify goals: Work with individuals to discover their dreams and desires.
* Coordinate support: Initiate and manage services that help people achieve their goals, including reablement and enablement opportunities.
* Promote independence: Use strength-based approaches to encourage choice, control, and self-directed support.
* Collaborate with partners: Work alongside health colleagues, statutory and voluntary organisations, and other professionals to provide comprehensive support.
* Show flexibility: Be prepared to work across different districts to meet community needs.
