An exciting opportunity has arisen for a practice nurse tojoin our friendly practice. The role includes the usual practice nurse dutiessuch as smear taking, wound care, vaccinations, pill checks and long-termcondition management.
The post holder will be responsible for the implementationof processes for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.
Working as a senior member of the practice multidisciplinaryteam, the post holder will ensure nursing services are delivered effectively tothe entitled patient population.
The Practice Nurse will be responsible for a number ofclinical areas such as infection prevention and control, chronic diseasemanagement, health promotion, well women and well man clinics, as well asactively supporting the practice management team in the reviewing and deliveryof clinical policy and procedure.
Main duties of the job
On occasion, there may be a requirement to carry out tasksnot listed below, that are within the reasonable scope of the role.
Duties will include but are not limited to:
* Undertake cytology
* Care of acute & chronic wounds
* Removal of Sutures/Staples
* Urinalysis for both acute & chronic conditions
* Contraception reviews
* ECGs both acute & routine
* Chronic Disease Management (Diabetes, Asthma and COPD)
* Hypertension Management 24hr Blood Pressure Monitoring andBP checks in clinic
* Adult & Childhood immunisations
* Prepare & send samples for lab testing
* Chaperoning
* Assisting clinicians with minor procedures
* Support the Infection Prevention Control (IPC) lead
* Assess the needs of patients ensuring the provision ofhealthcare is appropriate, incorporating evidence-based practice
* Implement & evaluate individual treatment plans forchronic disease patients that promote health and wellbeing.
* Identify, manage & support patients at risk ofdeveloping long-term conditions, preventing adverse effects to patientshealth.
* Provide routine nursing care to patients in accordance withclinical based evidence.
* To review patient medications to enhance compliance.
* Co-ordinatethe provision of travel medicine services.
* Ensure read codes are used effectively.
* Prioritise health issues and intervene appropriately.
* To support with any clinical emergencies.

Job responsibilities
Job summary:
Working in accordance with the Nursing & Midwifery Council (NMC) Code of Conduct, and The Royal College of Nursing (RCN) Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners across practice, as required.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
Duties and responsibilities:
Management of chronic disease:
* Identifying significant abnormalities
* Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Therapeutic monitoring:
* Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach
* Recognising abnormalities
* Identifying the impact of treatment and implementing or recommending changes as appropriate
Wound Care and management:
* Assessing routine wounds including trauma
* Assessing and prescribing to support good wound management in complex situations (e.g. use of Doppler technique)
* Identifying significant abnormalities
* Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
* Working with patients to develop a management plan where health problems or potential health problems are identified
* Recognising issues and gathering sufficient information to refer (e.g. drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)
Health screening:
* Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)
* Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
* Providing in depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
Travel health:
* Providing guidance in accordance with guidelines
* Identifying problems
* Provision of specialist and evidence-based support to the team.
Immunisation (adult and child):
* Working with patients with more complex immunisation issues
First contact:
* Working independently or alongside the general practitioner and making decisions/ recommendations as appropriate.
* To work alongside the Doctors to provide on the day treatment of minor illness.
Minor operations:
* Assisting the general practitioner and/or undertaking some simple procedures
* Providing routine ear care
* Dealing with more complex problems and prescribing if appropriate
Mental health:
* Communicating with key workers
* Administering appropriate prescribed therapies and monitoring for side effects
* Any other delegated duties considered appropriate to the post

Qualifications
* Registered General Nurse (with the NMC)
* Recognised qualification to diploma level or above
