An extremely exciting opportunity has arisen for a Patient Flow Coordinator to support the new and transformational One Coventry Integrated Team (OCIT) service. The trust along with Coventry City Council and Coventry and Warwickshire Partnership Trust have designed and developed 3 Local Integrated Teams (LITs) across the city of Coventry, which are supported by multi‑professional teams offering a holistic and integrated approach to urgent community needs for the residents of Coventry.

Responsibilities
* Deliver training in pathways and system support tools to support assessments as required, ensuring safe discharge planning and patient flow coordination with the correct identified level of support.
* Support patient flow for those patients case‑managed by Local Integrated Teams.
* Coordinate the assessments and referrals for patients related to patient flow.
* Communicate with patients, families, carers, and care homes within LIT processes and coordinate patient flow, planning meetings as required to support the Local Integrated Team.
* Work independently within a Local Integrated Team, seeking senior level of support as required on tasks outside of identified competency.
* Provide education, support and advice to the Local Integrated Team on the patient flow and discharge process and pathways.
* Represent Complex Flow Improvement, lead where applicable, provide team development and caseload updates to managers and inform LIT manager of blockages.
* Escalate and report any concerns via LIT manager to highlight delays in discharge.
* Establish and maintain effective communication with patients, carers, MDT, other workers and agencies across health, social care and the private sector.
* Resolve issues for patients and staff to ensure safe discharge and patient flow and elevate issues accordingly to LIT manager.
* Utilise IT systems to secure accurate, timely patient data. Enter data onto specialised databases and maintain equipment provision for audit purposes, within data protection guidelines.
* Communicate all relevant information about the patients within the LIT caseload to the multi‑disciplinary team and ensure accurate recording for handover of patients in your absence.
* Support staff and patients exposed to difficult situations arising in the clinical area (e.g. after receipt of bad news or following an unexpected event) and elevate to appropriate clinical lead within scope.
* Promote and monitor adherence to Health and Safety and Trust policy designed to protect healthcare staff and service users from known hazards.
* Work closely with care homes to facilitate flow to their normal place of residence.
* Maintain a clean, safe environment ensuring adherence to Trust standards of cleanliness, hygiene and infection control at all times.
* Ensure confidentiality of patient and staff information is maintained at all times.
* Promote awareness of the role of the Local Integrated Teams.
Qualifications
* Effective communication skills.
* High level of administrative skills.
* Eye for detail with a focus on service coordination and data collection.
