The Rehabilitation Assistant (RA) will deliver rehab to a group of patients under the guidance and supervision of a registered allied health professional in the Maximising Independence team.
The Maximising Independence team aims to provide a high quality, responsive rehabilitation service helping adults to regain their independence following illness or injury, support them with their Long-Term conditions, and promote health and wellbeing.
The team comprises Occupational Therapists, Physiotherapists, Speech and Language Therapists, Senior Rehab Assistants, and admin Assistants.
The team is based at Battersea Studios in Wandsworth and offers home-based sessions to patients in the Wandsworth locality.
Main duties of the job
* Work as part of a multidisciplinary therapy team delivering a community-based rehabilitation service under supervision of qualified therapists.
* Support patients as a lone support worker.
* Deliver rehabilitation programmes in patients' homes or care homes, including personal care, meal preparation, mobility, and exercise tasks.
* Provide clinical and non-clinical support to qualified therapists.
* Become proficient and competent in your area of practice through competency assessments.
* Collaborate with colleagues across community teams, GPs, social services, and voluntary sectors to optimize patient care and outcomes.

Qualifications
* Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognized equivalent.
* Willingness to undertake further training.
* GCSE grade A-C or equivalent in English and Maths.
* Registration with the national regulatory body (when available).
* PSI qualification – Trusted assessor training.
* ECDL certificate.
Experience
* Experience in health or social care settings.
* Experience working as part of a team.
* Experience with Microsoft Office and databases.
* Experience in community care and elderly care.
* Experience with NHS patient information systems (EMIS).
Skills and Knowledge
* Good numeracy and literacy skills.
