Main Duties
* Integral member of the Multi-Disciplinary Team under community mental health practitioners.
* Promote and monitor physical and mental health of service users.
* Support users in identifying, accessing and engaging support in local community.
* Carry Carers Assessments and work with users and carers with assessed eligible social care needs.
* Expected to work under minimal supervision with adherence to Trust procedures and policies.
* Responsible for detecting, alerting and responding to safeguarding concerns.

Person Specification
Essential
* Education: NVQ Level 3 or equivalent mental health experience
* Skills and: Ability to work with patients individually or group basis focusing on recovery pathway; Empower patients; Excellent verbal and written communication; Support multidisciplinary team; Recognise stress; Work autonomously and as part of team; Demonstrate awareness of communication style impact.
* Knowledge: Mental Health issues relevant to adults with acute and chronic mental health issues and forensic histories; Impact of mental health difficulties on service user life.
* Previous experience: Relevant experience working with adults with acute and chronic mental health problems.
Desirable
* Foundation Degree in Mental Health or social care.
* Experience of devising groups and activities.
* Working with multi-disciplinary team environment.
* Working with service users and carers within their own homes.
* Working in a role which includes physical health observations.
* Previous experience of venepuncture and taking ECGS.
