Pertemps is working with a supportive and forward-thinking Discharge to Assessment Team in Merseyside to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals to ensure a smooth transition for patients from hospital to home or other appropriate settings.
Adults Social Worker Role Overview:
As part of this innovative service, you will play a crucial role in assessing the needs of adults, facilitating discharge planning, and providing ongoing support to individuals and their families.
Key Responsibilities:
* Conduct comprehensive assessments of patients' needs and circumstances.
* Develop and implement person-centered discharge plans in collaboration with multidisciplinary teams.
* Provide emotional support and guidance to patients and their families during the discharge process.
* Ensure compliance with relevant legislation and best practice guidelines.
* Maintain accurate case records and documentation in accordance with organisational policies.
Candidate Requirements for the Adults Social Worker role:
* A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
* At least 3 years’ post-qualifying experience
* Current registration with Social Work England
* Experience in adult social care, hospital discharge or D2A services
* Strong communication and interpersonal skills, with the ability to work effectively in a team
* Knowledge of relevant legislation, policies, and procedures related to adult social care
