Meridian PCN is looking to recruit an enthusiastic and motivated Clinical Pharmacist to join our expanding pharmacy team. The post-holder will be working collaboratively with the pharmacy team to implement a safe and effective medicine management system within the practice. They will be responsible to manage long term conditions, provide advice and support to patients and colleagues, and to improve treatment outcomes with medicines. The post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice(s) and the PCN.
They will be working across two surgeries – Harston Surgery and Orchard Surgery Melbourn.
This is a hybrid role – the post-holder will be expected to be working at the surgery, as well as remotely, depending on needs of the business.

Primary Responsibilities
The following are the core responsibilities of the clinical pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a. Face-to-face or telephone consultations:
· Long term conditions management (eg hypertension, diabetes, respiratory) and optimise patients' medications where appropriate
· Carry out structured medication reviews for high-risk/complex patients (e.g polypharmacy, high dose opioids)
· To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home,
· To provide advice and support for patients with minor illnesses and self-limiting conditions
· To reconcile medicines following patients discharge from hospital or clinic appointments
· Review medications for newly registered patients
b. Medication Safety:
· To manage high risk medications monitoring and recall of patients
· Review all safety alerts ensuring the required actions are taken
· Review and implement safe prescribing systems for vulnerable patients, improving compliance
c. Acute/Repeat Prescribing:
· Encourage cost-effective prescribing within the practice
· Implement and embed a robust repeat prescribing system
· To reauthorise medication for repeat prescribing within scope of practice
· Review and implement safe prescribing systems for vulnerable patients, improving compliance
d. Actively refer/signpost patients to the correct healthcare professional
e. Medicines Information:
· To provide answers to medication queries from GPs, other health professionals and patients
f. Support practice to achieve QOF/DES/CQC targets as required
Secondary Responsibilities
In addition to the primary responsibilities, the clinical pharmacist may be requested to:
a. To carry out and evaluate clinical audits as part of ongoing service improvement
b. Provide education on medicines for the clinical team
c. Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
d. Act as the practice point of contact for all medicines related matters, establishing positive working relationships
Person Specification – PCN Clinical Pharmacist
Essential Qualifications:
* GPhC Registered Pharmacist
Desirable Qualifications:
* GPhC Independent Prescriber Qualification
* Completed CPPE Primary Care Pathway (or equivalent)
Essential Experience:
* Minimum of two years working as a pharmacist
* Experience in General Practice
Desirable Experience:
* Broad knowledge of General Practice and Primary Care
* Broad knowledge of secondary care
Essential Skills:
* Excellent communication skills (written and oral)
* Strong IT skills
* Clear, polite telephone manner
* Good time management
* Competent in the use of Office and Outlook
* Ability to promote best practice regarding all pharmaceutical matters
