As an integral member of the multi-disciplinary team, the post-holder will work in partnership demonstrating compassionate caring behaviours to fulfil the key functions of the North East Glasgow CMHT. This post is based at Parkhead hub, Community Older Adults Mental Health Team, however the post holder will, when necessary, provide cross cover support to the work of Belmont Resource Centre.
Shift pattern - 09.00- 17.00hrs
The post-holder will be responsible for the assessment, formulation, implementation and evaluation of complex programmes of care using a strength based recovery focused approach to care that proactively involves the person and optimises self-management.
The post holder is required to undertake duties without direct supervision as well as supporting and supervising work undertaken by registered and unregistered nursing staff.
The post-holder is responsible for proactively promoting partnership working with patients, carers and professionals/ agencies in line with legislative frameworks, agreed standards and clinical policies. This involves working in a variety of home/ near to home community environments.
Participating in as well as providing regular caseload line management supervision, clinical supervision and undertaking line management responsibilities are critical aspects of the role. Supervising the practice of others, monitoring and undertaking mentorship/ preceptorship responsibilities.
Supporting the team leader in the ongoing learning and development of the staff team and the CMHT key functions are important and expected aspects of this role.
Mental Health Nurse (RN3/ RNMH) level 1 NMC registration Evidence of:
* Experience of mental health nursing practice at Band 5 or above.
* Ongoing professional development.
* Specialist assessment skills, in particular those relevant to older adult mental health.
* Coordination of care • Knowledge and demonstration of evidence based psychological therapy skills.
* Awareness of Stress and Distress in Dementia interventions. • Line management responsibilities.
* Applied leadership skills. • Excellent team working skills.
* Enhanced communication skills. • Enhanced listening and interpersonal skills.
* Ability to work autonomously and delegate appropriately.
