We have an exciting fixed -term opportunity to join the Home Environment Assessment and Response Team (HEART) as a Case Progression Officer (CPO). HEART plays a vital role in ensuring that residents across Warwickshire have access to grants and facilities to adapt their homes to support living in an accessible, healthy, and safe environment. This role is as a CPO in the HEART team. The HEART team can work remotely at home, or in a HEART office. Sometimes, there is a requirement to attend an office or other location for work related activities such a team meetings or training, and this could be in the North or South of Warwickshire. As a CPO, you will be responsible for managing a diverse case load, consisting of a wide variety of cases that have already had an assessment completed and then require adaptions to be completed. The role of the CPO is to progress the case to facilitate the required adaptations, this includes requesting quotes from contractors, completing forms with customers, problem solving, updating a case management system, and liaising with external partners. You will be responsible for ensuring that your cases progress in a timely and efficient manner, in accordance with HEART policy. You will be able to manage competing demands and be flexible to meet the needs of the service. This role is an excellent opportunity to work with a team that consists of Occupational Therapists, Housing Assessment Officers, and Home Improvement Officers.
