The Home First Service has an exciting opportunity for an experienced Healthcare Support Worker (HCSW) to join our team to work across Berwickshire & Cheviot Localities.
Home First is focused on collaborative working with acute and community teams to support people home or within a homely setting. With a locality‑led community approach to reablement and broader health and social care services, the emphasis is on prevention, anticipation, and supported self‑management.
The Home First service is delivered in an integrated way alongside Hospital at Home, District Nursing, Community Allied Health Professionals, Adult Social Work and Adult Social Care services to provide a holistic community response across the Borders.
We are seeking a motivated and adaptable HCSW who would like to be part of a friendly and supportive team with an ethos of continuous improvement. The post holder must be confident in decision making and be able to work without direct supervision.
You will be part of a multidisciplinary team working alongside Registered Nursing staff providing the delivery of high‑quality person‑centred care to people in their own homes. You will also work with the other health professionals within the team, patients and families to coordinate care.
Successful candidates will be expected to support service delivery across the other localities that includes Central, Teviot & Tweeddale when required.
The role requires you to have a full UK driving licence with business insurance and be flexible to work weekdays and weekends on a rotational basis to meet the needs of the service. Hours of work are 08.00 – 20.30 over 7 days.
As from 1st April 2026 the Agenda for Change full‑time working week will be reducing from 37 to 36 hours per week. Part‑time hours will be reduced pro‑rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
