Overview
Our Out of Hours Service is a crucial part of our Adults Specialist Services delivering care to people who need it urgently or those with longer-term conditions. We are recruiting Band 5 Community Nurses to help us deliver the best experience possible in our North Somerset Team. As the South Wests largest provider in community healthcare, our multi-disciplinary teams of nurses, therapists, clinicians and support colleagues help people remain safe and live as independently as possible after injury, illness or hospital admission. We take a holistic approach involving people in their care, enabling them to thrive in their homes and local communities. In this role, you will focus on collaborative, safe, effective and evidenced-based practice. Working closely with primary and community services as well as with social care partners, you play a critical role in ensuring a person-centred experience.
Responsibilities
* Assess, plan, implement and evaluate individual, patient-centred interventions drawing on a wide range of specialist knowledge gained from practical experience and specific training.
* Be competent in a broad range of clinical skills including catheterisation, pain management, wound care, end of life care and IV medication after specific training and more.
* Actively use and promote shared decision-making, empowering people to improve self-management of their long-term condition while working collaboratively with our teams and system partners
* Supervise colleagues including; peers, students and unregistered workforce in nursing competencies, helping us shape our existing service and contribute to the future design and delivery.
