Contract type: Full time, 40 hours per week (Monday to Friday)
Rate: £40,000 per annum, plus £3,000 car allowance
Join us as our new Clinical Nurse Assessment Specialist at Exemplar Health Care.
Reporting to the Senior Clinical Assessment Specialist, you’ll work flexibly as part of a motivated Clinical Assessment Team, supporting our specialist care homes to respond to referrals.
We have an ambitious growth plan to expand our portfolio of homes. You will demonstrate passion, commitment and determination to ensure every referral is managed to the highest standard and our customers receive an exemplary experience.

About the role
No two days will be the same. Your day-to-day responsibilities will include:
* Support care homes to complete clinical pre-admission assessments, travelling nationwide within the allocated working day.
* Produce individual cost per care assessments that demonstrate the specialist care package, the justification for interventions and costs, completed during working hours (office or remote).
* Coach Clinical Nurse Managers and Home Managers in assessment and costing, and contribute to training delivery.
* Support processing of referrals, in conjunction with the Referrals and Occupancy Management Team.
* Uphold the company vision and Exemplar Health Care strategy.
* Support reassessment of existing service users where required.
* Build relationships with new and potential referrers.
* Attend conferences, events and networks to raise Exemplar Health Care’s profile as needed.
About you

* An ability to conduct thorough assessments of individual needs and identify suitable packages of care to achieve desired outcomes.
* Excellent verbal and written communication with all levels of staff and stakeholders.
* A passion for working with individuals with complex health needs.
* A desire to work in a fast-paced, responsive team to develop a systematic approach to referral management and ensure high-quality customer experience.
* Flexibility to work independently and as part of a team; good organisation skills and self-motivation.
* Willingness to travel to various portfolio homes.
* Confidence when representing Exemplar Health Care at internal, commissioner and contract meetings.
* First-level nursing registration with an NMC PIN (RN, RMN, or RNLD). We welcome Registered Nurses from all backgrounds (Adult, Mental Health and Learning Disability).
