York and Scarborough Teaching Hospitals NHS Foundation Trust is looking to recruit an enthusiastic and self motivated Discharge Liaison Officer.

You will be part of the Patient Flow team, working across the acute medical, surgical or elderly wards to optimise discharges and support patients and staff to negotiate discharge plans.

The Discharge Liaison Officer works closely with; ward staff, Social Care, Continuing Health Care and the voluntary sector.

Therefore you must be able to demonstrate collaborative working, as well as being able to build effective relationships with colleagues from other healthcare professions.

This role entails working closely with a wide multidisciplinary team, to facilitate the timely discharges of patients from the hospital environment.

You will benefit from working within a small, friendly, experienced, pro-active team who are looking for a colleague with a positive approach, and has a flexible and patient centred approach.

Please note you will be expected to join a weekend rota for this post.

The ability to escalate concerns via appropriate channels of escalation, is also essential in this role. The successful candidate will be required to use a variety of computer applications, and should be confident in IT skills. Qualifications

Essential criteria

11. Educated to GCSE Level or equivalent. English at C or above – or equivalent qualification/demonstrable experience.
12. NVQ Level 3 or equivalent experience.

Experience & knowledge

Essential criteria

13. Knowledge and understanding of the ward routine and processes with regard to discharging a patient
14. Knowledge and understanding of the differing needs of service users with a range of clinical conditions and potential disabilities
15. Demonstrable experience of adapting own approach to engage with service users, a range of external and internal stakeholders and colleagues at different levels of seniority and discipline
16. Experience of dealing with conflict and resolving effectively
17. Demonstrable experience of working autonomously
18. Evidence of effective problem-solving skills with the ability to be a flexible thinker under pressure
19. Knowledge of establishing and maintaining robust administrative processes and systems

Desirable criteria

20. Experience of working in a health or social care setting
21. Awareness of community services and partner agencies
22. Knowledge and awareness of community partners and agencies

Skills

Essential criteria

23. Effective negotiator and influencer with a range of stakeholders within and outside the Trust
24. Experience to work across a range of IT systems
