Health Care Assistant (Fixed Term Until June 2026)
Collaborating closely with our Community Nursing Teams, you will support seniors to provide care to patients in their homes, including diabetes, wound and end‑of‑life care. This role is crucial in preventing unnecessary hospital admissions while offering flexible working with state‑of‑the‑art tablets.
Responsibilities
* Assist in providing nursing care as delegated by the Team Leader within patients’ homes.
* Maintain knowledge, skills and attitudes necessary to deliver a quality service responsive to changing patient needs.
* Provide clerical and administrative support to the team as delegated by the Senior Sister/Team Sister.
* Deliver care in accordance with organisation policies, procedures and guidelines, according to competencies.
* Report any changes in patient condition to the Team Leader or, in their absence, the designated nurse in charge of the caseload.
Qualifications
* NVQ level 2/3 or equivalent qualification and relevant experience.
* Valid driving licence to travel to service users’ homes in West Lancashire.
* Strong communication, organisation and flexible approach.
* Ability to use IT systems, including Microsoft Office (Outlook, Word, Excel).
