An exciting opportunity has arisen to join the inpatient diabetes team at Liverpool University Hospitals NHS Foundation Trust, based at The New Royal site.

This post will be part of the team providing diabetes inpatient support to patients and staff across The New Royal, Broadgreen, and Clatterbridge Liverpool sites.

The team supports individuals with diabetes admitted to these sites, offering education and training to staff across all locations.

As a Band 6 Diabetes Inpatient Specialist Nurse, you will be part of a large Diabetes MDT, providing education and support to patients admitted to The Royal Liverpool Hospital, Broadgreen, and Clatterbridge Cancer Centre Liverpool.

This role involves supporting patient and carer education for inpatients, participating in diabetes rolling education programs, and in-reach teaching. You will serve as a source of specialist diabetes knowledge for staff across the sites.

Your responsibilities include supporting safe and timely discharges and referring patients to appropriate care post-discharge.

Role of the Band 6 CNS in Diabetes

* Perform specialist assessments of people with diabetes, mainly inpatient, planning, implementing, and evaluating care using advanced clinical skills.
* Undertake the diabetes competency program based on national TREND Diabetes competencies.
* Lead clinical care by managing a patient caseload, providing expert assessment, planning, and evaluation with MDT support.
* Coordinate and manage patient reviews from referral systems.
* Contribute to a multi-professional approach for ongoing care and discharge planning.
* Support patients and families with information to enable self-management and safe discharge.
* Assist and encourage the MDT in reviewing complex cases, developing innovative models of case management.
* Maintain own professional development and portfolio in line with re-registration requirements.
* Participate in supervision and teaching of students, new staff, and junior medical staff, including in-reach education.
* Contribute to educational program development related to diabetes care.
* Identify training needs and develop resources for nursing staff to support role development.
* Participate in diabetes awareness programs and national activities to promote education and safe care.
