Main duties of the job
We are looking for ANP and offer a salary £50.000 to £60.000 dependent upon experience.
To work as part of the clinical team dealing with a wide range of presenting health care needs in a primary care setting. Working with colleagues to ensure the highest standards of care for patients.
About us
We are a busy town centred practice. The team currently consists of 9 GPs, 2 ANPs, nurses, a HCA and pharmacist.
We also have the support of PCN staff in the form of a pharmacist, pharmacy technician, physiotherapist, social; prescriber, mental health nurse and community matron with CCT team.
There is on site parking.

Main Purpose of Post:
To assist medical personnel inthe care of practice patients, to include, treatment, preventative care,screening and patient education.
Job summary:
Responsible for attending to patients who wish to be seenby a health care professional; diagnosing and treating patients presenting withminor illness seeing and advising people in respect of their continuing medicaland nursing needs. These patients will have booked appointments at ten-minuteintervals. There is a requirement to see extra or emergency patients. The nursepractitioner will need to prioritise and triage the needs of patientsaccordingly making any necessary referrals for investigations in theappropriate manner. Patients with problems needing referral to secondary careshould be discussed with the registered GP before making such referral.
Duties and Responsibilities of Post
Provide, assessment, screening treatment, services and health educationadvice.
Assist with the organisation and co-ordination of the provision ofnursing services for the practice.
Provide nursing treatment to patients in participation with generalpractitioners using agreed protocols, including the taking of cervical smears.
Provide general and specific health screening to the practice patients(within the agreed protocols) with referral to general practitioners asnecessary.
Be competent in administration of childhood immunisation and travelvaccination.
Be competent and confident in managing people with long term healthconditions
Assess, diagnose, plan, implement and evaluate treatment/interventionsand care for patients presenting with an undifferentiated diagnosis
Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating bothdrug-and nondrug-based treatment methods into a management plan.
Prescribe and review medicationfor therapeutic effectiveness, appropriate to patient needs and in accordancewith evidence-based practice and national and practice protocols, and withinscope of practice.
Work with patients in order to support compliance with, and adherenceto, prescribed treatments.
Provide information and advice on prescribed or over-the-countermedication, medication regimes,side-effects and interactions
Support patients to adopt health promotion strategies that promotehealthy lifestyles, and apply principles of self-care.
Implement and participate in vaccination and immunisation programmesfor both adults and children.
Utilise and demonstrate sensitive communication styles, to ensurepatients are fully informed and consent to treatment
Recognise and work within own competence and professional code ofconduct as regulated by the NMC
Produce accurate, contemporaneous and complete records of patientconsultation, consistent with legislation, policies and procedures.
Prioritise, organise and manage own workload in a manner that maintainsand promotes quality.
Deliver care according to NSF, NICE guidelines and evidence-based care
Maintain and control injectable and oral drug stocks in the treatmentroom and doctors surgeries.
Advice to manager regarding health and safety aspects. Pathological specimens and investigatoryprocedures
Undertake the collection of pathological specimens includingintravenous blood samples, swabs, smears, etc.
Prioritise own workload and ensure effective time-management strategiesare embedded within the culture of the team.
Ensure accurate notes of all consultations and treatments are recordedadequately on the computer.
Assist in the formulation of practice philosophy, strategy and policy.
