Overview
Provide clinical support, expert care and clinical leadership to support patients with a diagnosis of colorectal cancer at ASPH receiving SACT. To provide expert, comprehensive knowledge and specialist advice, information and psychological support to patients with a diagnosis of a colorectal cancer receiving treatment. Positively impact the experience of patients and carers by providing high standards of clinical practice and care as appropriate across the trust.
Responsibilities
* Deliver specialist oncology nurse led clinics assessing fitness for treatment, delivering pre chemotherapy education. Able to support PSFU surveillance.
* Promote high standards of specialist care for patients with a diagnosis of colorectal cancer, ensuring a quality experience.
* Provide specialist support and care to patients admitted with complications of their cancer treatment.
* Work autonomously to support patients with colorectal cancer facilitating necessary investigations and providing clinical engagement and support for patients receiving SACT.
* Deliver HNA and treatment summary clinics for all patients.
* Be a resource both within the trust and the community for clinical advice and information.
* With the Lead Cancer Nurse contribute to clinical governance within the trust for cancer patients.
* Deliver care in line with National and Alliance guidance and requirements.
* Plan, deliver and evaluate programmes to address patients\' complex health needs.
* Initiate and promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist area.
* Discuss investigations and treatment for cancer with sensitivity, knowledge and expertise.
* Deliver clinical care and management in line with local and national guidance and best practice.
