* The post holder will work as an autonomous expert practitioner with responsibility for leading a team of nurses, providing highly skilled & developed quality nursing care for adults in their own homes and other community settings.
* To develop, implement and evaluate a seamless Hospital@Home (H@H) Service, ensuring that patients receive the highest standard of clinical care.
* To work as an autonomous practitioner with responsibility for facilitating early discharge, reduce length of stay and prevent hospital admissions.
* To provide continuing responsibility for the assessment, development, implementation and evaluation of nursing care for patients.
* To work in partnership with members of the multi -disciplinary team to maintain high quality care & promote practice that is patient centred.
* To continue to develop and maintain close links between Community and Hospital based services.
* .To be responsible for providing support to junior team members through developing their skills & completing staff appraisals. They will motivate staff to provide high standards of care by acting as a role model.
* To undertake the organisation and support of student placements & mentorship through their practice placements.
* To undertake and implement research and audit activities to ensure evidence-based practice and best practice guidelines are delivered within the service.
