Overview

An exciting opportunity has arisen within the Telford and Wrekin Community Mental Health Services for a Band 5 Occupational Therapist. This post includes a 12-month preceptorship programme designed to support your development into a Band 6 role.


Responsibilities

* Conduct occupational therapy assessments to identify care needs, contribute to the formulation of individualised care plans, and deliver evidence-based non-pharmacological interventions aligned with the persons goals.
* Work collaboratively within a multidisciplinary team, providing therapeutic input to support individuals in achieving their agreed outcomes.
* Coordinate and/or participate in the review of progress using recognised outcome measures, ensuring the involvement of service users and their identified carers throughout the clinical pathway.
* Manage a developing caseload of increasing complexity, undertake specialist assessments, care planning, and deliver non-pharmacological interventions to maximise independence and quality of life.
* Contribute to quality assurance through auditing and monitoring practices, complete risk assessments, and promote positive risk-taking as appropriate.
* Take responsibility for managing a caseload that increases in complexity with progression, including specialist assessment, care planning with service users and carers, and evaluation of individual treatment plans.
* Participate in setting quality standards, including auditing, monitoring and reviewing practice in line with clinical guidance, practice, and policy.
* Assess patient care needs, develop, implement and evaluate programmes of care; utilise outcome measures to review effectiveness of interventions as part of care planning and evaluation.
* Deliver a range of non-pharmacological interventions following occupational therapy assessments to maximise individuals 0 abilities and address unmet needs.
* Contribute to robust risk assessments and risk management plans; promote positive risk-taking to improve quality of life and occupational performance/independence.
* Maintain an effective reporting system by observing and recording patient conditions and ensuring accurate documentation.
* Support in the provision of formal and informal training of pre/post registration students, staff members, carers and service users.
* Participate in service development and uphold clinical governance standards; ensure care aligns with national and local guidelines; maintain professional competence and adhere to best practice frameworks.


Requirements

* Band 5 Occupational Therapist (12-month preceptorship) within the NHS; capable of developing into a Band 6 role.

The role includes participation in a duty rota/shift coordination system to ensure consistent service delivery across operational hours. Your role includes maintaining accurate clinical records, using outcome measures to evaluate interventions, and supporting the training of students, staff, carers, and service users. You will actively participate in service development, uphold clinical governance standards, and ensure care aligns with national and local guidelines. Maintaining professional competence and adhering to best practice frameworks will be essential throughout your role.
