Community Staff Nurse

Hampshire and Isle of Wight Healthcare Foundation Trust has exciting opportunity for an experienced Community Nurse to join the Petersfield & Bordon OR Waterlooville Community Care Team. Working in the community you will be providing care to patients with complex care needs, enabling patients to receive the care they need in their home.

We are looking for enthusiastic, caring & motivated professional with excellent clinical, managerial & communication skills, who can work autonomously, supporting the team lead to manage the case load and staff development.

The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.

Access to a vehicle and possession of a full driving license is essential.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

Person Specification


Qualifications

* BSc Nursing or RN currently registered with NMC.Practice assessor/supervisor.Post registration, specialist training relevant to speciality.Current (UK/EU) Driving Licence and access to own vehicle for work purposes is required.
