Frailty Practitioner

We are seeking an enthusiastic and experienced Band 6 Nurse to join our multidisciplinary team as a Frailty Practitioner. This role plays a key part in delivering high‑quality, person‑centred care for patients living with frailty, supporting early identification, comprehensive assessment and proactive management.
Responsibilities
* Deliver specialist frailty care and interventions, including assessment, treatment and management of a defined caseload as part of the multidisciplinary team.
* Undertake comprehensive assessments and implement evidence‑based care plans to meet the complex needs of patients living with frailty.
* Work across community settings, providing specialist clinical input to optimise independence, reduce hospital admissions and improve patient outcomes.
* Act as an autonomous practitioner while working collaboratively with GPs, therapists, social care partners and wider services to deliver coordinated care.
* Contribute to the development of frailty pathways and support service improvement initiatives, ensuring care is evidence‑based, safe and responsive to local needs.
Knowledge, skills and training
* Registered Practitioner.
* Evidence of recent professional development.
* Experience of working in community.
* Experience of managing change.
* Experience of working with adults in long‑term care facilities.
* Experience at band 6 level, or above.
* Able to use own initiative and appropriate risk taking.
Job specific experience
* Experience of managing change.
* Demonstrable knowledge of assessment and therapeutic interventions in area of specialism.
* Experience at band 6 or above.
* Recent previous experience within a comparable community role.
Managerial / Supervisory experience
* Experience of providing clinical supervision and mentoring to junior staff.
* Experience of supporting and supervising learners.
* Experience of devising and delivering training.
* Experience of using electronic patient / service user record systems.
Disclosure and Barring Service Check
