You will be based in Livingstone Hospital, Dartford, North Kent as part of our community nursing team who provide care to people in their own homes or within a clinic setting.
We empower our staff to work autonomously and with flexibility by providing them with state-of-the-art tablets to check in from home, write notes on the go, and avoid being confined to an office, allowing them to enjoy working in their community background.
Located just off the M25, with great access points and close to Bluewater Shopping and Leisure Centre, you can enjoy nearby coffee shops and food outlets.
As this is a community-based role, a UK driving license and access to a car are required.
Main duties of the job
Working closely with our Community Nursing Team, as a Healthcare Assistant, you will support senior team members in providing care to patients in their homes or in a clinic setting for conditions including diabetes, wound care, and end-of-life care. Our work aims to prevent unnecessary hospital admissions. You will deliver high-quality care, share best practices, and actively seek to improve our services.

Job responsibilities
This role is ideal for candidates with NVQ Level 3 or equivalent experience. Strong communication, organization skills, and flexibility are essential to deliver positive patient experiences throughout their healthcare journey.
You must be able to travel across the area promptly, so a UK driving license and access to a car are necessary.
Proficiency in IT systems and Microsoft Office (Outlook, Word, Excel) is required.
