Rehabilitation Assistant - Bradley Rehabilitation Centre
Salary - £19,657- £22,063 (Grade 5)

We are looking to recruit a Reablement Support Worker at Bradley reablement residential setting, working with vulnerable adults who have suffered an accident or short-term illness. You will teach them skills to enable them to perform everyday tasks and help them regain their confidence and independence. This is an incredibly rewarding role where you can gain valuable experience while helping to change someone's life.
This is a permanent, shift-based role, working 5 days out of 7, totaling 25 hours.
People stay at Bradley RC for short periods, typically between 3-4 weeks. During this time, they participate in planned rehabilitation plans developed by our on-site health therapist team before returning home.
Principal responsibilities:
1. Undertake individual care plans, providing personal, practical, and rehabilitative duties to maximize individuals' independence and well-being.
2. Carry out therapeutic activities and skills practice according to the plan to improve and maintain mobility, daily living skills, transfer, and confidence.
3. Encourage and support individuals to maintain independence in decision-making, coping strategies, social contacts, activities, and safe home environments.
4. Communicate effectively verbally, in writing, and via IT systems to ensure safe service delivery and accurate care plan reviews.
5. Observe, record, and report any changes in individual conditions promptly to senior staff, ensuring safety and well-being.
6. Research community resources, social activities, benefits advice, local services, and support groups, and signpost clients accordingly.
7. Implement equalities best practices to ensure fair and equitable service provision.
8. Maintain compliance with local policies and CQC standards, including ongoing training and personal development.
Skills & experience:
* Relevant Social Care qualification (NVQ Level 2 or equivalent)
* Training in medication administration procedures
* Training in Moving & Handling in People Producers
* Good communication skills with individuals, carers, relatives, and professionals
* Ability to use initiative when reporting concerns and acting on them
* Willingness to undergo a 12-week on-the-job induction and complete mandatory training
