Chineham Medical Practice Practice Nurse
Chineham Medical Practice is looking for a highly motivated registered nurse to join our existing Practice nursing team.
Job-type : Part-time, Permanent
Salary: Dependant on experience
Hours: 24 per week split over 3 days
Main duties of the job
The post holder will work as part of the primary healthcareteam providing Personal Medical Services to the patients of the Practice,including treatment room care and chronic disease management.
Assist inand perform routine tasks related to patient care as directed by senior nursingstaff and GPs
To assessthe needs of patients ensuring the provision of healthcare is appropriate,incorporating evidence-based practice
To identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to patients health
To requestpathology services and process pathology results as required
Chronicdisease management: Diabetes, COPD, Asthma and Contraception
To ensurecompliance with the practice chaperone policy
To assistGPs with minor surgery when required
Toprioritise health issues and intervene appropriately
To supportthe team in dealing with clinical emergencies
To provide arange of nursing services, including cytology, wound care, doppler, travelservices, CHD/stroke monitoring, baby clinics, vaccinations and immunisations,health checks, suture removal, assist GPs with minor surgeries and coilfittings etc.
To ensureread codes are used effectively
Comply withQOF, enhanced services and medicine management initiatives to maintain highstandards
Any otherdelegated duties appropriate to the post
