Overview
Senior Physiotherapist (Band 7) role at Central London Community Healthcare NHS Trust. Based with Brent Urgent Community Response Team. This is a seven days a week service, 365 days a year, working from 8:00 to 20:30 or 10:00 to 22:30. Weekend and bank holiday work is required; aim is to work approximately one weekend in four. The service brings hospital-level care to patients’ homes to avoid unnecessary hospital admissions for adults aged 18 years and over.
Join a multidisciplinary team to provide integrated and personalised care to patients, working alongside therapists, doctors, dietitians and pharmacists to promote the best possible patient care and to mirror the standard of care patients would receive in secondary care.
Responsibilities
* Undertake comprehensive head-to-toe assessments independently to determine patient need/risk, plan, implement and evaluate care with support from the service consultant.
* In the absence of senior colleagues, take charge of the unit, manage day-to-day team activities, take handovers and contribute to treatment planning at virtual ward rounds with the consultant.
* Work in partnership with multidisciplinary team members, other professionals and agencies to meet health and rehabilitation needs of Brent Urgent Community Response Team patients.
* Maintain clinical standards using an evidence-based approach within the service, incorporating clinical governance principles and values.
* Ensure effective communication; provide support, supervision and clinical expertise to all team members.
* Develop skills toward a holistic approach to patient care.
* Take on extended roles such as venepuncture, drug administration, advanced assessment of acutely ill patients and acute injury assessment where appropriate.
Qualifications and Requirements
* UK HCPC registration
* Full, valid UK driving licence with access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
