What you will do:
* Conduct comprehensive assessments and design individual care plans for elderly patients in the emergency and frailty departments.
* Focus on acute discharging, ensuring safe and seamless transitions for patients back to their homes or other care facilities.
* Collaborate closely with multidisciplinary teams to deliver holistic and patient-centred care.
* Support the inpatient care of the elderly, addressing their specific needs and promoting independence.
Requirements:
* Hold HCPC registration to validate your professional competence.
* Minimum of 2 years experience within a similar healthcare setting to bring depth to your role.
* Car driver preferable to ensure mobility across various assignments easily.
