About the role:
As a Team Leader you'll lead the care team on shift, ensuring safe, person-centred care for our residents and acting as the key point of contact for families and professionals.
Key responsibilities
* Lead and coordinate the care team on shift; allocate tasks and manage breaks.
* Deliver high-quality, person-centred care and support with daily living.
* Administer and record medications safely (eMAR), escalating concerns promptly.
* Complete and update care plans, risk assessments and daily notes accurately.
* Carry out observations (e.g., BP, pulse, weight) and report changes to nurses/management.
* Conduct effective handovers and maintain clear communication across the team.
* Mentor, supervise and coach new starters and Care Assistants; support training compliance.
* Liaise professionally with residents' families, GPs and external professionals.
* Uphold safeguarding, dignity, infection control, GDPR and health & safety standards.
* Assist with audits (care records/meds), incident reporting and falls prevention
