The post holder will work with Mental Health in a community setting, managing their own diary and caseload of service users. They will work as part of a multidisciplinary team, carrying out duties autonomously and using their initiative, demonstrating a commitment to a recovery approach and working collaboratively with service users.
The successful candidate will serve as a care coordinator for service users with various mental health issues, providing high-quality, recovery-focused care in the community, and promoting social inclusion. They will collaborate with services within LPT and the voluntary sector to support patient recovery.
The role involves chairing regular Multi-Disciplinary Team (MDT) meetings, utilizing clinical knowledge and assessment skills to support colleagues and evaluate new patient referrals, with decisions made collectively by the team. The team includes Psychologists, OTs, Consultant Psychiatrists, HCSWs, PSWs, and Community Connectors.
