Position: Care Navigator
Hours: Shifts available Monday - Friday 08:00 - 18:30 (or half days 08:00 - 13:30 / 13:30 - 18:30
Location: Sefton, Liverpool
Connaught are seeking experienced Customer Service / Administrators with Healthcare experience to work as a Care Navigator playing a vital role in a busy NHS primary care environment, providing a high-quality, patient-focused service.
To be considered for this role, you must have relevant experience in a healthcare setting. This would be suitable for those with Receptionist experience in GP Surgeries or other healthcare settings, 111 Health Advisors or 999 call handlers.
As a Care Navigator, you will:
Handle incoming calls professionally, signposting patients to the appropriate clinician or service, and escalating emergencies as needed.
Offer operational support to clinical and non-clinical teams.
Log accurate information using EMIS Web, including follow-ups, referrals, and clinical instructions.
Provide compassionate support to patients and carers while maintaining discretion and professionalism.
Work flexibly to meet service needs, following standard operating procedures and maintaining patient confidentiality.
