To work closely with the Paediatric Diabetes Specialist Nursing Team to plan and prioritise workload and carry out delegated tasks within the remit of the post holders own sphere of responsibilities and competencies. To undertake all biometric data collection and perform review tasks as needed within the outpatient clinic setting. Assist patients to set up downloading and sharing their diabetes technology data to the relevant digital platforms. Record all findings in the clinical records and patient notes according to Trust policy.
Report any concerns, changes and variations in the patients psychological, physiological or social conditions. Empower children and young people with diabetes and their families/carers to manage their condition effectively by delivering education and health promotion as appropriate, with emphasis on patients who are having difficulty engaging with their health needs.
