About the role
Fixed Term Contract or Secondment for 9 months, full time 37.5 hours per week.
At our Trust, we are seeking a Cancer Pathway Navigator within the Colorectal Team, who will work with the Colorectal Triage and Colorectal Nurse Specialist.
Key Responsibilities
* Act as patient advocate, providing support and information throughout the entire cancer pathway to reduce patient anxiety.
* Reassure and provide information on appointments, interventions/tests, ensuring patients arrive for interventions fully prepared.
* Work closely with the MDT coordinator to support and coordinate the care of cancer patients through complex care pathways, ensuring all patients move through the pathway without unnecessary delays in order to achieve a definitive diagnosis by day 28.
* Track patients on cancer surveillance as part of the introduction of Patient Stratified Follow Up (PSFU).
* Work autonomously and demonstrate strong communication skills, commitment and compassion for patients and their quality of care and experience.
* Collate and input data, maintain accurate records across a broad range of IT systems.
* Deal with complex situations with service users and clinicians, both in primary and secondary care over the phone and in person.
Qualifications & Core Skills
* Excellent communication skills.
* Advanced IT, administrative, secretarial and organisational skills.
* Experience and knowledge of cancer pathways.
* Strong data collation and IT proficiency across multiple systems.
* Ability to work autonomously, with commitment and compassion for patients.
