PCN Clinical Pharmacist (Independent Prescriber)

Wouldyou like to join our friendly and resolute team of clinicians andnon-clinicians at a large NHS general practice based in Slough, Berkshire?

We are looking for a motivated and experiencedClinical Pharmacist to join our team. You will be part of a team of six ClinicalPharmacists (FRMG & PCN SPINE) providing multi-disciplinary support to thepractice and its patients through primary care. You would be based at Farnham Practice but would be expected totravel to our other locations and our PCN SPINE partner, Kumar Medical Centre,within Slough, to be involved in the wider runningof the practice, such asparticipating in clinical meetings and contributing to the development of newservices. You will be part of the Clinical Pharmacist team ensuring prescribingmedication is safe, effective, and is optimised for and with our patients. Main duties of the job

Towork closely with SPINE PCN GPs and act as point of contact in the practice forall medicines related queries for health care professionals and patients.

To manage the repeat prescribing reauthorisation process and undertakemedication reviews of patients, some of which may need to be undertaken in thepatients home.

To facilitate good working relationships between the practice and CommunityPharmacies and Hospital Pharmacies.

To support the practice in delivering on Prescribing Indicators, PrescribingBudget, QOF and Locally Commissioned Services.

Manage practice formularies to improve the quality, safety, and costeffectiveness of prescribing.

Where appropriate, review pathology results for patients on known medicines.

To help plan, develop and support the introduction of new working processeswithin the practice to optimise the quality and safety of prescribing andundertake medication audits.

Farnham Road Medical Group (FRMG) started as a small surgery and havegrown over the years into an outstanding large training GP practice that isapproved by Health Education England. With four clinical sites based in Slough,we have a large Multi-Disciplinary Team (MDT) looking after approximately38,000 patients. The practice is part of SPINE Primary Care Network (PCN)andare committed to innovation in providing high quality health care for ourpatients and a protected working and learning environment for our staff. Our MDTteam includes GPs, pharmacists, nurses, physician associates, paramedics, MSKpractitioners, Mental Health Practitioners, managers, social prescribers, andcare coordinators supported by a large Patient Services team. This resoluteteam provides an evolving healthcare and medical service to meet the varyingneeds of our patients and, in turn, provide what we believe is a sustainablemodel of modern General Practice. Job responsibilities

Working closely with SPINE PCN GPs- Ips to resolveday-to-day issues relatedto medication.

To act as point of contact in the practicefor all medicines related queries for health care professionals and patients.

To manage the repeat prescribing reauthorisation process by reviewing requestsfor repeat prescriptions and medicines reaching review dates.

To undertake medication reviews of patientsincluding those with multiple long term conditions and polypharmacy to improve patientoutcomes and maximisecost-effective prescribing. Some reviews may need to be undertaken in the patientshome.

Ensure that the practice repeatprescribing policy is regularly reviewedand that it is beingadhered to with the aim of reducing waste andimproving safety.

Contribute to reductions in medicine-related hospitaladmissions and readmissions by supporting patientsto get the best outcomes from their medicines and identifying andaddressing medicines-related issues.

To support patientsrecently discharged from the hospitalby ensuring they understand their medication, any changes that have happened recentlyand have a timely supply.

To facilitate good working relationships between the practiceand Community Pharmacies and Hospital Pharmacies.

To follow up issues highlighted by Medicines Use Reviews carriedout in local Community Pharmacies.

To support the practice in delivering on Prescribing Indicators, Prescribing Budget, QOF, LocallyCommissioned Services and the QIPP agenda.

To undertake minor ailment clinics or triage.

Manage practiceformularies to improvethe quality, safetyand cost effectiveness of prescribing.

Log and actiondrug withdrawals and alerts e.g. monitoring CAS and MHRA alerts aimed at improving medicines safety.

Where appropriate, review pathology resultsfor patients on known medicines.

To qualify as a prescriber if this has not alreadybeen done.

To help plan, develop and support the introductionof new working processes within the practice to optimise the quality and safetyof prescribing.

To produce patientinformation leaflets and posters and run medicine awareness projects throughoutthe year.

To attend the CPPE trainingand development program.

Awareness of and compliance with all relevantpractice policies and guidelines.

Reporting on Significant Events.

Contributing to the development of computer-based patientrecords on EMIS Web.

Contributing to the summarising of patient recordsand SNOMED-coding patientdata.

Participate in regular staff training sessionsto ensure development of skills and knowledge; to assess own learningneeds and undertake learning as appropriate.

Participate in clinical, multi-disciplinary and whole team meetings.

Observe and comply with allappropriate statutory requirements in relation to Child and Adult Safeguardingand to be familiar with the reporting process should his be required.

Contribute to policy development and implementation of procedures and guidelines in relation to their own area of work.

Attending any Protected LearningTime sessions providedby either the CCG for the Practiceor by the Practice itself.


Person Specification


Experience

* To be a qualified Clinical Pharmacist with a minimum of two years experience in the Pharmaceutical Industry.
* Excellent interpersonal and communication skills
* Ability to work independently and as part of a team and performed effectively under pressure.
* To be part of the planning, developing, and supporting the introduction of new working processes within the practice to optimise the quality and safety of prescribing.
* Be highly motivated with excellent clinical timekeeping.
* Ability to consistently observe and comply with all appropriate statutory requirements in relation to Child and Adult Safeguarding and to be familiar with the reporting process should this be required.
* Contribute to the summarising of patient records and SNOMED-coding patient data.


Qualifications

* Qualified Pharmacist and registered with GPhC
* Enhanced DBS check in required.
