Your Role:
5. Coordinate discharge planning for patients.
6. Work with multidisciplinary teams on individualised plans.
7. Communicate with patients and families about post-discharge care.
8. Ensure compliance with hospital policies.
Requirements:
9. Degree in social care
10. Social Work England Registration
11. Experience in Hospital Discharge team
