We are offering an exciting opportunity for a NMC Registered Band 6 Senior Nurse Co-ordinator to join our Discharge to Access, Home First Service.
Working pattern is 5 days over a 7-day shift pattern.
The Home First Team provides clinical support for community patients to either avoid potential hospital admissions or support people to return home through the Discharge to Assess Pathway following a period of acute care in hospital.
We pride ourselves on the delivery of quality care to patients, many with complex needs. We actively promote the philosophy of self-care and promotion of independence whilst offering rehabilitation and assessment. The ethos of the team is to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their home environment so they can remain at home.
As an efficient and effective team member you will work collaboratively in a multi-disciplinary team alongside the Operational Leadership to ensure high quality care delivery, provide specialist assessments and care planning to our patients in conjunction with service users and carers.
You will be required to work shift patterns covering a 24/7 365-day period. You will also be required to travel independently over a large geographical area.
As a Senior Nurse Co-ordinator you will assess, manage, plan and deliver care whilst providing support to your peers and team leader.
As part of this role, you will also so be expected to:
Triage referrals for the discharge to assess pathway in line with the referral criteria.
Provide holistic complex assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue Viability, Palliative Care.
Supervise pre-registration students.
Work collaboratively and actively supporting and liaising with other health and social professionals and agencies.
Act as a role model and motivate team members in working towards an innovative and high standard of practice.
Participate in the development of an effective team and the development of productive working relationships throughout the Trust.
