We are looking for a Band 6 Mental Health Practitioner to work within OP Courage Veterans Mental health and Wellbeing Service Core team for Humber and North Yorkshire
We are looking for a practitioner who can show a commitment to the particular needs of this client group and an understanding of the key issues, including psychological trauma, that are often experienced by veterans.
OP Courage Veterans Mental health and Wellbeing Service Core team for Humber and North Yorkshire focuses on the assessment, signposting and case management of serving personnel leaving the armed forces, as well as those veterans already living in the community. We also have a integrated psychological therapy pathway as part of the service
The post holder will provide specialist and expert assessment and formulation for veterans and transitioning military personnel presenting with complex mental health, substance misuse and social care issues, as well as signposting to relevant services/agencies and offering advice and consultation on clients' psychological care to multi-disciplinary mental health staff, third-sector agencies and other, non-professional carers. The post-holder will also be responsible for providing psychological support, working alongside existing veteran and NHS mental health services, where appropriate.
Main duties of the job
To undertake screening and telephone assessment duties including liaison with referrers and other service providers to ensure clients are treated in the most appropriate service for their needs.
To conduct a thorough psycho-social assessment of clients referred to the Service, to identify their needs and develop a collaborative care plan.
To provide case management and brief interventions for clients with moderate and complex mental health problems where required focus is generally around risk to self or others, safeguarding children or safeguarding vulnerable adult.
To assess clients presenting with multiple and complex issues and make decisions regarding the appropriate therapeutic intervention required to meet client's needs. This may include referral on to another service or modality of treatment.
To assess risk of harm to self/others or property and take action as appropriate. This may involve breaching of client confidentiality in the interest of the clients or others safety e.g. Safeguarding Children/Adults, acts of terrorism etc. This will be assessed in line with Trust policies
To organise and manage a clinical caseload.
To maintain appropriate electronic records using a web based clinical system in line with agreed protocols as directed by Line Manager.
There is an expectation for you to deliver services across Humber and North Yorkshire region. This may include home visits.

Person Specification
Qualifications and Knowledge
* Diploma or Degree in Professional qualification equivalent to RMN, Registered Social worker, OT or equivalent AHP
* Registration with the HCPC/NMC or equivalent
* Full understanding/application of relevant clinical practice/standards/audit within identified clinical area
Experience
* Appropriate knowledge/experience of working in the relevant service area
* Understanding of caseload management clinical responsibilities
* Evidence of specialist practice skills and able to demonstrate the impact of this on practice change/development
Skills and Competencies
* Evidence of good time management skills
* Ability to manage daily operations and maintain a safe working environment
