Job Summary
We have an exciting opportunity for an urgent care practitioner to join our well-established, high-performing group of practices. The role involves working as part of a team with other urgent care practitioners and a Duty Doctor to help patients access urgent on-the-day support, including daily visits to patients' homes. Our Clinical Team is supported by a multidisciplinary team that enables you to do what you do best: care for your patients. We are committed to patient care and are looking for a dedicated professional who shares our values and passion for delivering quality care. The post is permanent and full time, though we may consider part-time applicants and there is some hours flexibility.
Main duties of the job
As part of our Practice Team, you will provide care to patients from initial history taking and clinical assessment through to diagnosis, treatment, prescribing and evaluation of care. You will be confident in clinical triage and face-to-face consultations, using your knowledge to investigate, diagnose, treat and refer registered patients within set guidelines. As an advanced clinical practitioner we are seeking the following experience:
* A relevant Nurse Practitioner or Paramedic Qualification to Masters Level or equivalent
* An Independent Prescribing Qualification
* Previous minor illness experience

Job Description
Job responsibilities
* To support the daily on-call team to provide an on-the-day emergency service for patients. The role includes assessment, diagnosis, treatment and/or referral of patients with undifferentiated and undiagnosed conditions. The post holder will hold a prescribing qualification and will work within their level of competency.
* To provide assessment, diagnosis and treatment at first point of contact by attending to patients autonomously within the GP surgery, their own homes and nursing or residential homes.
* To support and continue the development of on-the-day urgent patient service in the Practice
* To accurately triage and prioritise patients, including in acute situations, using techniques to elicit the history of an event/illness and past medical and drug history
* To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral
* To undertake and interpret diagnostics and take appropriate action to manage patient care, using broad clinical guidelines (NICE, etc.)
* To signpost appropriately to other services in the identified healthcare pathway
* To refer patients to appropriate care settings within locally agreed pathways
* To discharge or treat, as appropriate and within the scope of practice
* To maintain accurate documentation of patient interactions and care, including adverse events
* To advise and support the public on healthy lifestyles, health promotion and disease prevention
* To provide clinical advice to other team members where appropriate
Person Specification
Essential
* A relevant Paramedic Qualification
* An Independent Prescribing Qualification
* Previous minor illness experience
* Driving Licence and means of travel between Practices
* Excellent communication, interpersonal and emotional intelligence skills
* Ability to adapt to service needs while working in a busy environment
Desirable
* Experience of working in General Practice
