Key Responsibilities
Promote effective self‑management of patients and provide professional intervention for community management to prevent unnecessary hospital admissions, improve quality of life, and identify patients who would benefit from case management.
Lead the organisation and management of the service, including resource planning, caseload management, benchmarking, clinical audit, provision of respiratory education, and teamwork development.
Contribute to patient and professional education related to the management of respiratory disease in Waltham Forest.
Provide advice and information for the development of the respiratory service in Waltham Forest.
Hold caseload and carry out home visits or clinics as appropriate.
Main Duties of the Job
1. Undertake comprehensive assessment of patients with respiratory disease, particularly asthma, including those with complex presentations and those at end of life, and formulate individualised management and treatment plans using a wide range of therapeutic options.
2. Undertake a holistic, unified assessment process in partnership with all agencies to prevent acute admission where possible and deliver services to meet patients’ needs.
3. Interpret and analyse clinical and non‑clinical data to form accurate diagnoses and prognoses for patients with complex respiratory disease, recommend the best course of intervention, and develop comprehensive management plans.
4. Monitor patients, evaluate and modify treatment to measure progress, and ensure effective intervention in collaboration with appropriate health and social care professionals, maintaining a seamless approach to care across all agencies.
5. Keep accurate, up‑to‑date records of patients and communicate timely, informative reports and summaries for other agencies (e.g., GPs, consultants, MDTs, social services).

Qualifications
* NVQ Level 2 or equivalent standard of literacy and numeracy
* Nurse/Prescriber/Non‑Medical Prescriber
Experience
* Minimum 3 years experience as a Band 6 nurse (RGN) in a complex management role
* Experience in staff, process, and finance management
