Job overview We are looking for an enthusiastic and motivated Band 6 Community Dietitian to join our supportive Community Dietetics Team. This role is ideal for someone eager to develop specialist skills in the dietary management of Type 2 and gestational diabetes while making a real impact in the community. You will manage your own caseload, providing high‑quality 1:1 consultations across outpatient, community and remote settings, as well as delivering engaging group education sessions. Strong communication and behaviour‑change skills will help you empower patients to better understand and self‑manage their diabetes. We are seeking a Dietitian who is keen to learn, proactive in improving practice and passionate about enhancing patient experience. You will contribute to service development through audit, evaluation and use of patient feedback, and work closely with diabetes specialist colleagues and the wider MDT. If you are compassionate, proactive and ready to make a real difference to people living with diabetes, we would love to hear from you. Advert Responsible for a defined clinical case load of adult patients, with Type 2 diabetes and gestational diabetes who may have other clinical conditions and needs. To be a confident dietetic practitioner for patients seen in outpatient clinics, in their own home or in care homes. Provide advice by telephone and other digital services such as video consultations as appropriate to patients. Provide group education sessions to patients with diabetes and other medical conditions and work with other health professionals to ensure advice is appropriate to meet patient’s needs. Communicate complex and sensitive information effectively with patients, families and carers to ensure understanding of condition and enable patients to self-manage their diet. This requires persuasive, motivational and counselling skills to overcome barriers to change. Liaises with community teams, diabetes specialist team and staff to provide an integrated approach to patient care. Identifies areas for improvement or efficiency and actively participates in the development of the service to patients in the community and to diabetes care. Contributes as a team member of the department and participates in department developments, clinical governance issues, clinical audits, projects and professional development meetings.
