This role helps people with long‑term respiratory conditions receive safe, effective care at home, making a real difference to their quality of life.
You’ll work as part of a supportive community respiratory team, alongside specialist nurses, GPs and other health professionals. Your work will support people who need home oxygen therapy, as well as those living with complex respiratory conditions, including palliative and end‑of‑life care. You’ll see patients in their own homes and in clinics, helping them stay independent and well for longer.
What you’ll do:
* Carry out holistic respiratory and home oxygen assessments in the community
* Manage a caseload and review care in line with national and local guidance
* Support people and carers with self‑management, education and treatment plans
* Work closely with multidisciplinary teams to plan and coordinate care
* Maintain clear, accurate records using electronic systems

* A registered nurse with post‑registration experience in respiratory or long‑term condition care
* Confidence working independently and making clinical decisions
* Strong communication skills and a calm, empathetic approach
* Willingness to keep learning, with support through training and supervision

Essential criteria
* Be a registered Nurse.
* Evidence of ongoing professional development
Desirable criteria
* Demonstrate you have completed a related respiratory course
* Demonstrate you have completed a diagnostic and consultation course.
Knowledge
Essential criteria
* Demonstrate excellent consultation and assessment skills.
* Demonstrate excellent communication skills with patients as well as team members and other staff.
* Demonstrates the ability to work independently using own initiative.
* Demonstrate knowledge of oxygen therapy and its application.
* Demonstrate an up-to-date knowledge of long-term condition management.
* Demonstrate a basic ability to use Word, PowerPoint, e mail, Excel (or similar spreadsheet or database packages).
Desirable criteria
* Demonstrate knowledge of guidelines regarding long term conditions and oxygen therapy as well as other relevant guidelines.
* Demonstrate knowledge of arterial blood gas analysis.
Experience

* Significant post registration experience
* Demonstrate working within a relevant field of respiratory care.
* Demonstrate experience working within long term condition management.
* Demonstrate experience working autonomously when undertaking patient care
* Demonstrate experience of teaching patients and a variety of Health Care Professional’s.
