Overview
We are advertising posts in our well established CESR programme at the University Hospitals of Derby and Burton NHS Foundation Trust. The CESR programme is expected to require 3–5 years for completion in most cases. The contract is initially for 12 months and will be reviewed annually and extended on a mutually agreed basis, subject to successful completion of required competencies at annual review. The posts provide access to support and training required to complete all aspects of the CESR process, with potential secondment-style modules to address relevant educational and competency needs.
The post will usually rotate between Royal Derby Hospital (RDH) and Queen’s Hospital Burton (QHB) to provide a breadth of training. For significantly less-than-full-time positions, base location at RDH can be negotiated, with occasional cross-site working depending on service need.
Main duties
Duties will be split between winter pressure services and other training needs throughout the year. This may include Orthogeriatrics, mental health liaison or surgical liaison. The timetable below is flexible and can be tailored to the successful applicant’s training needs:

The post will participate in an out-of-hours rota supporting acute medicine and the HOOH service at the QHB and RDH sites.

Job description and responsibilities
Clinical Activities
Inpatient
RDH: 4 base wards and outliers/winter ward; QHB: 2 base wards and outliers/winter ward
Provide senior ward cover with leadership of the winter ward board rounds (Mon–Fri) and daily senior ward reviews when consultants are unavailable. Support other DME wards when the base team SpRs are on-call or on leave.
Referrals
Provide at least one session weekly supporting the liaison service and inpatient referrals. Increase activity when possible to provide adequate CESR-training exposure. Outpatient clinics: when not on winter pressures, provide one falls/frailty clinic per week.
Clinical duties include leading the management of acutely ill frail older patients. Leadership development will be supported through supervision of junior doctors and involvement in their education, both formal and informal.
The post holder should prioritise and time-manage effectively and utilise senior staff for advice. Completion of work-based placed assessments to document development is encouraged. Education is a key part of the role, with involvement in training all levels of staff (medical, nursing, AHPs). Regular attendees include Student Nurses, Doctors, ACPs, Therapies, and Pharmacy students.

Eligibility
* Full GMC Registration with a licence to practice at time of application
Desirable and Essential criteria
* Commitment to Continuing Professional Development and clinical governance
* Experience in supervising and teaching resident doctors and medical students
* Evidence of relevant academic, research achievements and leadership skills (desirable)
* Experience of clinical audit/QIP and applying evidence-based practice
* MBBS or equivalent; MRCP or equivalent higher qualification; Life Support Qualifications (ALS)
* Completed at least four years of postgraduate training (two in Medicine) or equivalent
* Minimum 1 year NHS experience (desirable)
* Up-to-date knowledge of Medicine for the Elderly and its application (essential)
* 360-degree feedback and patient survey feedback (desirable)
