As a Health care support worker you will work as part of the planned care community nursing service delivering care to people living in their own home environment. We will support you to develop skills that will enable you to carry out a variety of interventions, appropriately delegated by a clinician in the team.

The successful candidate will have experience of working with patients ideally in the community setting and appropriate NVQ or equivalent qualifications or experience.
Main duties of the job
To work as part of the Adult Community Nursing Services over a 7 day period. Ensuring high quality health care is delivered to patients in their homes as an integrated part of the mainstream Integrated District Nursing service.
Qualifications
* GCSE Maths and English
* NVQ Level 3 in H/S Care or equivalent
* Care Certificate or be willing to complete within 10weeks of start date
* Basic Wound Care
Experience
* Experience of working in an employed appropriate care role.
* Experience of working with patients with long term conditions
* Wound care
* Experience of working in a community care setting
