Job purpose
The main duties of the posts are to care for individuals with complex health care needs in their own homes within the community.
Role details
Band 3, Health Care Support Worker. Base: Blaenau Gwent. This post involves caring for a young girl transitioning from paediatric to adult care, living in the Blaenau Gwent area. Health Care Support Workers (HCSWs) should have knowledge and experience of working with patients with complex health needs. A learning disability background is desirable but not essential; HCSWs are welcome to learn these skills.
Successful staff will enable the individual to live a full and productive life, supporting physical and psychosocial needs within her family unit. The role offers the nurse an opportunity to develop a relationship with the patient to understand her individual health needs and provide high-quality nursing care. Training and development opportunities are available within the Care at Home Team and the learning disability nursing team, with a focus on innovative evidence-based practice to maximise quality of life for people with learning disabilities within the community.

Education
Essential criteria
* NVQ Level 3 in Direct Care or equivalent or working towards obtaining
Desirable criteria
* NVQ Level 2 in Direct Care
Experience
Essential criteria
* Experience of working at Band 2 or equivalent in a health or social care setting
Desirable criteria
* Undertaking clinical duties e.g. bowel management
* Experience of working with individuals with complex needs
Skills
Essential criteria
* Able to make informed decisions and act on own initiative
* Ability to keep calm in stressful situations
* The ability to drive patient vehicles and travel between sites in a timely manner
