Community Nurse Section 117 reviewer out of area (Mental Health and Learning Disabilities)
Tri-borough - Hammersmith and Fulham, Kensington & Chelsea, and Westminster.
The job role includes the following:
* Reviewing Section 117 aftercare plans for patients placed out of area, following an agreed process where responsibility sits with the ICB.
* Working closely with clients, families, local authority services, and placement providers.
* Collaborating on creating crisis and contingency plans.
We are looking for:
* A clinician with experience or strong interest in working with people with learning disabilities and mental health challenges.
* A clinician who keeps the client at the center of their practice.
* A strong understanding of the Section 117 aftercare process.
* A dedicated, motivated individual capable of working independently with team support.
Could this be you?
The main duties involve reviewing Section 117 aftercare and care plans for clients residing outside the tri-borough area, working closely with clients, support networks, care providers, and local authorities. You will assess mental health and learning disabilities, identify strengths and risks, and develop tailored aftercare plans.
You will provide updates to the ICB lead, raise concerns or positive feedback, and support in creating crisis and contingency plans.
We are looking for someone with experience in:
* Putting the client at the center of practice
* Working with people with learning disabilities
* Working within mental health
* Understanding Section 117 review process
* Understanding the Dynamic Support Register
* Knowledge of Positive Behaviour Support
This role offers flexibility to be based in Hammersmith & Fulham, Kensington & Chelsea, or Westminster, with travel across the UK. You should be able to work independently and organize your time effectively.
Clinical supervision will be provided by the Trust Consultant Nurse for Learning Disabilities, and you will be part of the Learning Disabilities and Autism Clinical network. Management supervision is provided by the LD Psychiatry service manager.
Applicants may have contact with patients or service users. We encourage vaccination to protect yourself and others.
Responsibilities include reviewing S.117 aftercare plans for Tri-Borough patients living outside the area, following an established review process, and developing supportive aftercare plans. You will work with clients, families, and care services, providing expert advice in learning disabilities to support this cohort of patients.
Maintain high standards of care, ensuring practices are informed by the latest evidence-based research.
